Second level Index Entry (<index.entry>).

Use:

Indexes may be generated automatically from Index Entry (<index.entry>) elements embedded in the text. An Index Entry (<index.entry>) element is just a placeholder, it does NOT surround words in the text. The words needed to generate the index are carried in the level ("level1", level2, "level3", and "level4") attributes. Each Index Entry (<index.entry>) contains all the appropriate levels.
